New Patient
Your First Visit
At your first dental visit you can expect a comprehensive examination to assess your oral health, including a review of your medical history and discussion of dental concerns or symptoms you may be experiencing.
This will include a thorough inspection of your teeth, gums and overall oral structures. Digital X-rays may be taken to identify any underlying issues not visible during the visual examination. The dentist will discuss the findings with you, address any concerns, review your oral health habits and develop a personalized treatment plan if necessary.
Additionally, you may receive professional dental cleaning to remove plaque and tartar buildup. We encourage you to ask questions or communicate any anxieties or specific needs that you may have during this visit.
New Patient Form
Welcome! To ensure that we provide you with the best possible care we kindly ask you to complete the following new patient information form. Your responses will help us understand your medical and dental history, any specific concerns you may have and enable us to tailor our services to meet your individual needs.

Reducing Anxiety
Reducing anxiety at dental visits is important for maintaining good oral health and making the experience more comfortable. Here are some tips to help alleviate dental anxiety:
-
Share your fears and concerns with the dentist before the appointment. They can tailor their approach to make you feel more at ease.
-
Arrange a non-treatment appointment in advance to meet the dentist, discuss any fears and familiarize yourself with the surroundings. This can help reduce anxiety on the day of treatment.
-
Work out a signal, such as raising your hand, to indicate when you need a break during the procedure. This can give you a sense of control over the treatment.
-
Practice deep breathing, meditation, or other relaxation techniques before and during the appointment.
-
Listening to music or an audiobook can be a good distraction. It helps to dampen the sounds of the dental equipment and can be a source of comfort.
-
Inquire about sedation options, such as oral sedatives and or IV sedation.
-
Having someone you trust with you to provide emotional support and reassurance during the appointment.
-
Don't hesitate to ask for breaks. Short breaks can help you feel more in control and reduce anxiety.
Insurance Billing
At our practice we strive to make the financial aspects of your dental care as transparent and convenient as possible. To facilitate smooth processing of insurance claims please review the following information.
Insurance Coverage
Please provide us with accurate and up-to-date information regarding your dental insurance coverage. This includes the insurance provider's name, policy holder's name, policy number and group number. If you have dual coverage kindly provide information for both plans.
Verification of Benefits
Before your appointment our team will verify your insurance benefits to help you understand the coverage provided by your plan. We will do our best to estimate the portion of the treatment cost covered by insurance but please note that this is an estimate and not a guarantee of payment. Sometimes your insurance company may not share information with the office and we will request the information directly from you.
Co-Payments and Deductibles
Any applicable co-payments or deductibles determined by your insurance plan will be collected at the time of service. We accept various forms of payment including cash, credit cards and cheques.
Billing Process
We submit insurance claims on your behalf to ensure prompt processing. The remaining balance will be billed to you. Please be aware that certain services or treatments may not be covered by your insurance and that you are responsible for any uncovered charges.
Dual Coverage
If you have dual coverage, please inform us of both insurance plans. Coordination of benefits will be applied to maximize your coverage. It is important to note that not all dental procedures are covered by insurance, and you may be responsible for any non-covered services.
Claim Disputes
In the event of an insurance claim dispute or discrepancy our receptionist will be available to assist you in resolving the issue. Feel free to contact our office for clarification or additional information.
Payment Policy
Payment in full, for any part of the services that are not covered by your dental insurance, is due when services are rendered. Acceptable forms of payment include cash, personal cheques, Visa, Mastercard and Debit.
